Position Summary
Another Chance is seeking a highly organized, client-centered Case Manager to support individuals enrolled in our substance use treatment programs. This position focuses on removing barriers to treatment engagement, supporting stabilization, and strengthening recovery capital.
The ideal candidate is proactive, resourceful, and understands the importance of coordination in behavioral health systems.
Key Responsibilities
- Conduct needs assessments related to housing, employment, medical care, insurance, and legal obligations
- Coordinate with probation officers, DHS, housing providers, and community resources
- Assist clients with insurance navigation and benefit activation
- Support discharge planning and transition of care
- Maintain accurate documentation in the electronic health record
- Participate in staffing and treatment team collaboration
- Advocate for clients while maintaining appropriate professional boundaries
Qualifications
- Experience in behavioral health, addiction treatment, or human services preferred
- QMHA, CADC, or equivalent experience preferred but not required
- Strong organizational and follow-through skills
- Ability to manage multiple client needs simultaneously
- Clear and professional communication skills
Preferred
- Knowledge of Washington County and Multnomah County resources
- Experience working with justice-involved populations
- Familiarity with Medicaid and OHP systems
